Covid19 vaccination screening questions


 

Have you been invited to attend for COVID-19 vaccination?

Please read the following questions before you book as we aim to identify anyone who might not be eligible for the vaccine before attending.

1. Have you already had a COVID-19 vaccination?

You shouldn’t book an appointment as we are only booking first vaccinations at the moment. 

2. Have you had any other vaccination in the 7 days before your appointment?

You should wait until 7 days after your last vaccination before having your COVID-19 vaccination. 

3. Have you been unwell with fever in last 24 hours?

You should wait until you feel well before you book your COVID-19 vaccination. 

4. Have you ever had any serious reaction to any other vaccine?

You should contact the surgery for advice before booking your appointment. 

5. Have you ever had an unexplained anaphylaxis reaction (severe allergic reaction requiring hospital admission or adrenaline injection)?

You should contact the surgery for advice before booking your appointment. If you know what you are allergic to e.g. wasp sting, then you are OK to book appointment. 

6. Are you, or could you be, pregnant?

You should contact the surgery for advice before booking your appointment.  COVID vaccination is not currently recommended routinely in pregnancy. 

7. Have you taken part in a trial of a potential COVID-19 vaccination?

You should contact the surgery for advice before booking your appointment. 

8. Do you take warfarin?

Do you know your INR and it is stable and below 4? If not please contact the surgery for advice before booking your appointment.

9. Do you have a bleeding disorder?

You should contact the surgery for advice before booking your appointment. 

10. Do you have current symptoms of COVID-19 or have you tested positive for COVID-19 in the last month?

You shouldn’t book an appointment if you currently have symptoms of COVID-19.  If you’ve tested positive for COVID-19, you should wait for 4 weeks before booking your vaccination appointment.
